Abstract

[extract] The Palliative Care Outcomes Collaborative (PCOC) is a national quality initiative to support continuous improvement of palliative care in Australia. PCOC focuses on supporting services to measure service and patient level outcomes, and to use the data derived from these assessments to drive continuous quality improvement.
